Vocational rehabilitation application
1. Obtain a Medical Statement B that states the physician's opinion on your work ability and chances for rehabilitation. 2. File an application (+ a plan, if you already know what your rehabilitation will consist of).
Vocational rehabilitation plan
Make a plan if 1) you have submitted a rehabilitation application and 2) you have received a preliminary decision concerning the right to rehabilitation but you do not have a plan yet.
For persons in rehabilitation
Notification of studies, vocational training
Submit a notification of vocational training immediately when your studies begin, and at the start of each term, in August/September and in January. You can submit the form at the earliest on the day your studies begin.
Notification of studies, apprenticeship
Submit a notification of the start of your apprenticeship immediately when the theory portion of your apprenticeship begins. After this, please notify us whether your apprenticeship agreement will continue at the start of each new (autumn or spring) term.
Write up a commuting invoice if you are applying for compensation for expenses related to studying, work trials, job coaching or visits to a rehabilitation partner.
